If you run the New Project Wizard and make a simple application, do breakpoints work correctly in that application? Another way to tell is to look in the registry at HKCR\CLSID\{0A29FF9E-7F9C-4437-8B11-F424491E3931}\InprocServer32 : if there is nothing there, then all managed breakpoints are broken and click No above, but if there is something there then click Yes.
